Medical coding is the act of turning health care procedures, diagnoses, equipment and medical services into a universal medical alphanumeric code. Medical professionals use this code to document patient medical records, correctly bill patients for medical procedures and create claims to be paid by insurance companies. Medical coding processes occur every time an individual visits a health care provider and it's at the health care provider's discretion which code to use for each visit. This allows the health care provider to not only keep track of patient visit history but also to receive payment for treating patients. The following are the primary documentations that are typically converted into standardized code by a medical coder:
- Treatment
- Services
- Supplies provided to the patient during a visit
- Diagnosis
- Medical condition
- Unusual circumstances related to a patient's medical condition
- Necessary services, treatments or supplies for a patient to be treated.
